Job Opportunity in Aged Care



We are seeking a dedicated and experienced individual to join our team as a Clinical Aged Care Assessor. As a Clinical Aged Care Assessor, you will play a vital role in assessing the needs of older Australians and ensuring they receive the right care.



Key Responsibilities:

You will be responsible for conducting comprehensive assessments of clients to identify their complex needs and refer them to appropriate aged care services under the Aged Care Act. Your day-to-day tasks will include collaborating with clients, families, and multidisciplinary teams, working collaboratively with colleagues to promote a supportive team culture, conducting accurate aged care assessments using the Integrated Assessment Tool (IAT), reviewing support plans to meet client's changing support needs, and maintaining currency of knowledge relating to aged care services.



Requirements:

To be successful in this role, you will need to have a tertiary qualification in a health-related discipline directly related to Health, Aged Care or related specialist area, such as Registered Nurse, Medical Officer, Occupational Therapist, Physiotherapist or Social Worker. You must also have current unrestricted registration with the Australian Health Practitioners Regulation Agency (AHPRA), an Australian Driver's license, and access to a registered, insured vehicle. A minimum of one year demonstrated experience in Australia or Overseas delivering services in Aged Care settings and/or to Aged persons including people living with dementia is also required.
