**About the Role - Social Worker**

The Star Care Group, one of Western Australia's leading providers of community and in-home disability support, is currently seeking an experienced **Social Worker** to join our dynamic and diverse team. This position plays a vital role in assisting The Star Care Group in delivering essential services to our participants while ensuring efficient service provision.

As a **Social Worker** at **The Star Care Group**, you will build strong working relationships with both clients and staff. You will serve as a key liaison, engaging through phone and face-to-face interactions to support client onboarding and ongoing support matters.

We are looking for someone who is energetic, solutions-focused, data-driven, well-organised, detail-oriented, and an exceptional communicator.

**To be successful: Applicants must have full membership with the Australian Association of Social Workers (AASW).**

**Key responsibilities of this role include**:

- Collect and review and prepare necessary documentation and information for client registration.
- Serve as the primary point of contact for clients during the intake process.
- Confidently manage the onboarding process for new clients and address onboarding inquiries independently in the absence of onboarding officer.
- Communicate effectively with clients to gather necessary information and address any inquiries.
- Actively participate in networking events to build professional relationships, exchange information on services provided.
- Ensure compliance with relevant regulations and company policies.
- Conduct internal audits and maintain compliance.
- Conduct house inspections to ensure clients maintain hygiene, uphold accommodation standards. Prepare property inspection report and report if any issues to management.
- Monitoring and reporting on the progress of clients and refer clients to relevant departments that can provide additional help.
- Work closely and communicate information with other departments, such as the intake team, rostering team, Service Manager, and Managing Director to ensure a seamless client experience.
- Perform additional duties as instructed by the Services Manager or Managing Director to support team objectives.
- Adhere to confidentiality and privacy policies when handling sensitive client information.
- Address and resolve issues or concerns raised by clients during the intake process.
- Escalate complex issues to the Services Manager.
- Maintain accurate and up-to-date client documents and records. Filing, managing documents, and performing other administrative tasks.
- Document any issues or challenges encountered during the intake process and provide recommendations for improvement.
- In case of any incidents, complete incident reports and update the incident register accordingly.
- Input client information into databases or customer relationship management (CRM) systems.
- Stay informed about industry regulations and best practices related to client intake.
- Conduct evacuation drill for both employees and clients.
- Prepare Support Plan Progress Reports.
- Update ongoing changes in the Support Plan of support workers.
- Attend meetings related to client intake, support plans, or other relevant topics.
- Document key points and action items from meetings for reference and follow-up.
- Other duties as required from time to time.

**Essential Selection Criteria**
- **Bachelor's or Master's degree in Social Work (completed).**:

- **Full membership with the Australian Association of Social Workers (AASW).**:

- Current NDIS Worker Screening Check.
- Australian Citizen or Full rights to work and live in Australia.
- Knowledge on SAT and ACAT processes (desirable)
- Current (or ability to obtain) National Police Check.
- COVID-19 vaccination.

**Skills and Attributes**
- Strong interpersonal and communication skills.
- Ability to manage and prioritise multiple tasks.
- High attention to detail and documentation accuracy.
- Problem-solving and critical thinking abilities.
- Experience working with diverse populations, particularly in disability and aged care.
- Ability to work independently and collaboratively in a team environment.
- Strong technical skills and proficiency with Microsoft Office products and data management systems.
- A proactive approach to the safety and wellbeing of yourself and participants.
- Understanding of professional boundaries, privacy, and confidentiality issues.
- Experience working with Mental Health would be advantageous.
