Our home support services enable older people to remain independent in their homes and communities longer. The service supports people aged 65 years and over and Aboriginal and Torres Strait Islander people aged 50 years and over who are experiencing difficulty with everyday tasks.
Key Responsibilities:
* Develop and review personalized care plans to enhance quality of life.
* Collaborate with the Aged Care Team to deliver exceptional services.
* Prior experience in planning, coordination, and delivery of services to older people who live at home is essential.
* Familiarity with the aged care and community services sector, regional service system, and local providers is required.
* Able to work respectfully and collaboratively with Aboriginal and Torres Strait Islander peoples, their families, communities, staff, and organizations.
