About the Role:

ACH Group have a full-time opportunity, within the Home Care Community program based at Milpara.

Effective delivery of the Social Links group experiences to enable customers to live Good Lives by connecting customers with relevant social experiences that match their interests. Whether customers have a passion for art, love to sing or enjoy fishing, there's an experience to match their interest.


What you'll do:

* Manage the planning and logistics for new and existing customers, applying strong leadership and innovation
* Provide time limited individual planning, support and coaching to customers around their social goals
* Administration, scheduling and compliance tasks
* Promote and grow the Social Links Programs
* Work collaboratively to plan and support customer participation
* Participate in co-design and evaluation utilising insights into customers' preferences


About you:

* Strong documentation skills with intermediate computer literacy
* Skills to motivate and engage individuals and enhance & develop abilities, skills and interests
* Excellent interpersonal and communication skills
* Hold a current driver's licence, that complies with Australian licencing laws and regulations, with access to a registered reliable vehicle
* Applicants that also hold Tertiary level education in health, community, or other relevant area or Certificate IV in Community Services/ Aged Care/ Service Coordination or working towards
