Collaborating closely with multidisciplinary teams, including ward NUMs, medical staff, and allied health professionals, you will coordinate and manage comprehensive care plans, ensuring timely referrals and seamless discharge planning. Your role will be pivotal in delivering age-appropriate care, promoting best practice, and improving patient outcomes across a variety of clinical settings. You will also act as a key liaison with external healthcare providers, residential care facilities, and community services, fostering strong relationships to support admission avoidance and ongoing care coordination.
