WANTED: Care Coordinator/Senior Support Worker

• Location: Sunshine Coast

• Hours: Permanent, Full-Time

About Us

TAUS Care is a multicultural in-home and community support service provider with a very positive reputation for supporting complex needs clients on the Sunshine Coast. We also support clients in Brisbane, Ipswich and the Gold Coast. We have a specific focus on supporting participants of the National Disability Insurance Scheme (NDIS) with the intention to expand into other sectors such as Aged Care in the future. Our diverse range of services enables us to provide end-to-end, holistic care and support as participants’ needs change. Our talented senior leadership team will provide you with significant mentoring, training and ongoing support so you can thrive in your Care Coordinator role.

We are currently looking for a Care Coordinator/Senior Support worker with previous experience supporting clients with complex disabilities.

Job Responsibilities

* Directly coordinate service delivery for your allocated clients.
* Provide direct support to participants with complex needs.
* Manage and supervise support workers who deliver services to clients.
* Be confident working with clients with complex needs, including people with behavioural issues and where Positive Behaviour Support Plans (PBSPs) are in place.
* Work with diverse client groups including people with a disability and their families/carers, people with mental illness, people who are homeless or residing in marginalised housing and who may be at risk of homelessness.
* Work closely with other Care Coordinators and HR to coordinate staffing to meet client support needs.
* Frequent travel within Sunshine Coast to meet client needs and occasional travel to the greater Brisbane region as required for training.
* Implement NDIS Practice Standards and TAUS Care policies and procedures with support workers.
* Electronic reporting of reportable incidents (including medication incidents and Restrictive Practices) using NDIS Quality Safeguards Commission portal.
* Collaborate on new client intake assessments with Operations Manager, if required.
* Collaborate regularly with client stakeholders and prepare frequent progress reports.

* Well-developed verbal, non-verbal and written communication skills.
* Excellent attention to detail.
* Demonstrated ability to manage a client caseload, incidents, progress notes and associated reports
* High-level knowledge and skills in the coordination of direct supports to assist people with disability in their community and/or in their home.
* Knowledge and understanding of the NDIS Practice Standards and adherence to the NDIS Code of Conduct.
* Highly developed written communication and interpersonal skills, with demonstrated experience in gaining the cooperation of internal and external stakeholders.
* Competent use of Microsoft Office Suite (including Teams).

Qualifications

* Additional basic and specialist training will be provided upon commencement in this role.
* (Desirable) Relevant Diploma or Degree in either human and/or community services, or social work, psychology, counselling, health science, nursing, occupational therapy or disability management.
* (Required) Yellow Card / NDIS worker screening clearance
* (Required) Blue Card (Working with children)
* (Required) Current First Aid and CPR (or immediately obtain)
* (Required) Driver’s License and comprehensive vehicle insurance and vehicle for work use
