ProCare Australia are supporting our valued Client to recruit for multiple Case Manager positions within Housing and Homelessness. We are recruiting experienced Case Managers available for an urgent start. The roles available are based in Ipswich.

Case Manager
The Case Manager positionprovides direct service delivery to support people at risk of homelessness through providing planned support, case management and advocacy. The aim is to maximise individuals and family's capacity to be independent, self-reliant and connected to appropriate social and community supports. Support is person-centred, proactive, reliable, comprehensive, flexible and practical that is delivered in a manner that is respectful of the dignity and privacy of individuals. The program will provide services to people in their own homes or outreach, intervening early to prevent a person from losing a tenancy that is at risk or to make a planned transition to alternative suitable housing.

To be Successful:
* Completed Bachelor Degree in Human Services, Social Work, or similar.
* Case Management experience is essential.
* Experience in Housing and Homelessness, Domestic Violence and / or Child and Family roles - Intake and Assessment / Case Work experience is highly desirable
* Possess the following attributes: optimism, kindness, accountability, flexibility, and the ability to work autonomously.

* First Aid Certificate
* Drivers License and own vehicle
* National Police Check (Valid within 12 months)
* Fully Vaccinated against Covid-19 (3 Doses - 2 x Vaccinations + Booster)
Salary:
* ProCare Australia Category 3 Locum Rates - $56/hour + 12% super
* Opportunity for contract extension or permanency following a successful locum at SCHADS level 5 - $100,783.28 – $105,349.92 per year + 12% Super + salary sacrifice
