What You'll Be Doing
* The Sustaining NSW Families (SNF) Plus pilot program is a nurse‐led, structured sustained health home visiting program for children and families from pregnancy up to 2 years of age.
* Provide specialist social work and counselling interventions as part of the SNF Plus multidisciplinary team.
* Focus on early intervention, that aims to promote child health and development.
* Help families work together to build stronger bonds and support parent health and wellbeing.

Qualifications & Experience
* A bachelor's degree in Social Work, eligibility for membership with the Australian Association of Social Workers (AASW)
* Minimum of 3 years clinical experience as a social worker
* Demonstrated clinical reasoning and expertise in social work practice relevant to attachment theory, the biopsychosocial model, social determinants of health and trauma informed practices with vulnerable and complex families
* Experience completing psychosocial assessments, delivering focussed psychological strategies and engaging families in targeted interventions, utilising trauma informed and child-centred practices
* High level of effective verbal, written and interpersonal communication skills; culturally appropriate and ensuring reasonable adjustments for CALD or disability backgrounds
* Experience working collaboratively with health staff, non-government agencies and service providers, ensuring child‐centred, therapeutic team working toward self‐determination in families
* Commitment to adhere to AASW Practice Standards, Code of Ethics, Continuous Professional Development policy and internal policies; support level 1 or 2 social workers through clinical supervision
* Active participation in planning, implementation, evaluation and reporting on programs; identify areas for improvement; lead quality improvement activities including research and in‐service education sessions
* Current unrestricted NSW Driver's Licence; availability for travel including overnight stays
