Clinical Nurse Specialist – Diabetes Services
A Clinical Nurse Specialist (CNS) in Diabetes Services is an advanced practice registered nurse with specialized expertise in the assessment, management, and education of patients with diabetes. This role involves delivering high-quality, evidence-based care to individuals with complex or chronic diabetes needs across inpatient, outpatient, and community settings.
You will provide clinical leadership, supports multidisciplinary teams, and works collaboratively with patients, families, and healthcare professionals to optimize diabetes management and improve health outcomes. Your responsibilities include developing and implementing individualized care plans, providing patient and staff education, monitoring treatment effectiveness, and contributing to quality improvement initiatives.
In this role, you will also promote best practices in diabetes care, supporting policy development, and ensuring compliance with clinical standards and guidelines. Strong communication, critical thinking, and a commitment to patient-centered care are essential.
You will be required to work Fridays, other days are negotiable.
Key Responsibilities:
* Deliver expert clinical care and consultation for patients with diabetes
* Educate patients, families, and healthcare staff on diabetes management
* Support complex case management and care coordination
* Lead and participate in quality improvement and service development initiatives
* Ensure adherence to clinical guidelines and best practice standards
