Clinical Care Coordinator
We are seeking a skilled and compassionate Clinical Care Coordinator to join our team in delivering high-quality evidence-based clinical care to adults experiencing mental health concerns. As part of our multidisciplinary team, you will play a crucial role in providing safe and supportive environments for individuals and their families.
Key responsibilities include:
* Conducting comprehensive intake and assessments, including risk assessments and care treatment planning
* Collaborating with internal and external services to ensure seamless care coordination
* Providing brief psychological interventions tailored to individual needs
To be successful in this role, you will require:
* Full AHPRA registration as an Occupational Therapist or Mental Health Nurse
* General or Provisional AHPRA registration as a Psychologist/Clinical Psychologist
* AASW registration/eligibility as a Social Worker (suitable for graduate Social Workers wanting to build skills in the mental health field)
