Geriatric Social Worker Role
We are seeking a dedicated and compassionate Geriatric Social Worker to join our team. The ideal candidate will have a strong background in geriatric care, long-term health care, or community services for the aging.
In this role, you will provide comprehensive social work services to our clients, including psychosocial evaluation and intervention, case management, individual and group counseling, and advocacy. You will be a key member of our interdisciplinary care team and will work closely with other healthcare professionals to ensure our clients receive high-quality care.
Responsibilities include:
* Presenting social work concerns and issues, suggestions for intervention, and recommendations for treatment plans to the interdisciplinary team.
* Conferring with clients and families regarding individual expectations and goals, attending family meetings, and coordinating care planning.
* Attending morning meetings, reviewing significant events, and making recommendations for interventions and care plan changes.
* Intervening in crisis situations, advocating for client needs, wishes, and rights to self-determination.
* Participating in the development and revision of client care plans as a member of the interdisciplinary team, integrating social work treatment plans into overall care plans.
* Assessing client, family, and home environments for areas where social work planning and interventions can be beneficial, maintaining required documentation in electronic medical records.
* Assisting in determining client and family needs related to social supports, psychological supports, financial supports, and housing options.
Requirements:
* Licensed by the NYS Education Department as a Social Worker.
* Master of Social Work degree from an accredited program.
* Minimum of one year professional experience in geriatric care, long-term health care, or community services for the aging.
* Excellent written and verbal communication skills.
* Demonstrated organizational, problem-solving, and conflict resolution skills.
* Strong interpersonal skills.
* Proficient in MS Office programs and able to learn new software rapidly.
