Employment Status: 2 x permanent, full time roles available
Days & Hours of Work: Mon - Fri, totalling 38 hours per week
Competitive Salary: Band 5 - $83,038.80p.a.+ 12% superannuation
Location: Council office in Carnegie, with some flexibility to work from home

About the Role

The Community Care Support Officer assists Team Leaders to lead a team of Support Workers to deliver a coordinated range of in-home client services as well as establish, monitor and review services in response to clients' changing needs.

You will be responsible for assisting Community Care Team Leaders in:

* The leadership, coordination, support and supervision of Support Workers
* The monitoring and review of ongoing and changing needs of clients and their carers
* Actioning referrals for services received from assessment services including completion of service priority of access, an agreed service plan, OH&S plan and confirm service fee/s


Key Selection Criteria

To be successful in this role, you will bring the following skills and competencies:

* Tertiary qualification in a management, health, allied health, social science or related discipline would be well regarded and/or significant experience working with older people in a community setting
* Knowledge and understanding of aged care and disability services and programs
* Intermediate skills in Microsoft Office applications
* Previous experience with client management and rostering software would be an advantage
* Current Victorian drivers licence
* An understanding of the needs of and issues affecting older people, people with disabilities and their carers
* The ability to communicate with older people, people with disabilities and their carers
* An ability to respond to staff and client issues in a logical and considered manner


Mandatory pre-employment requirements

* Be willing to undergo and complete a satisfactory criminal history check
* Provide or obtain an employment-based Working with Children check
