About the Role
We are looking for a compassionate and dedicated professional to join our Homelessness Youth Services team as a Contract Referrals and Centre-Based Practitioner. This role focuses on supporting young people experiencing homelessness by managing referrals, providing direct centre-based care, and connecting clients with essential services.
This is a fixed-term full time contract position, 38 hours per week, based in Beenleigh, QLD, until June 2026.
Responsibilities
* Receive and assess incoming referrals for young people experiencing homelessness.
* Conduct initial intake assessments and develop individualized care plans.
* Deliver centre-based services, including counselling, case management, and life skills support.
* Maintain accurate client records, ensuring compliance with organizational policies and regulatory requirements.
* Liaise with internal teams, community services, and external partners to ensure continuity of care.
* Provide guidance, advocacy, and support to young people, addressing immediate and long-term needs.
* Contribute to team meetings, case discussions, and ongoing professional development.
* Support the development of programs and services that meet the unique needs of homeless youth.
Qualifications, Credentials and Other Requirements
* Degree in Health, Human Services, Behavioural Science, Social Work, Psychology or similar, and/or equivalent practical experience.
* Current Queensland Driver Licence
* Current Blue Card (Working with Children Check)
* Demonstrated experience providing case management and assessment support to adults and young people experiencing or at risk of homelessness.
* Experience delivering intervention strategies relating to family support, homelessness, and at-risk young people.
* Strong written and verbal communication, negotiation and relationship building skills.
* Ability to work both independently and collaboratively in a team environment.
* Competent in Microsoft Office and comfortable learning new systems.
* Commitment to flexible working hours as required.

You have strong case management, advocacy, and support skills, a sound understanding of community and government resources, and the ability to work collaboratively with colleagues, families, and external agencies. You are committed to promoting independence and positive outcomes for young people, maintaining high professional and ethical standards, and engaging in continuous professional development while contributing to a supportive and positive team culture.
