Job Overview
We are seeking a highly skilled and dedicated case manager to join our team in Launceston with regular travel to Shearwater.

* Manage a client caseload, developing effective care plans that meet individual needs.
* Build strong relationships with clients, their families and healthcare professionals.

Key Responsibilities
* Case Management: Provide day-to-day management of client cases, including assessment, planning, implementation and review of care plans.
* Care Planning: Develop and implement comprehensive care plans that meet the needs of clients, taking into account their goals, preferences and values.
* Service Delivery: Deliver high-quality consumer-directed services that promote independence, dignity and well-being.
Requirements
* Relevant experience in case management, community services or aged care.
* Knowledge of Commonwealth Home Support Program (CHSP) and Veterans' Home Care (VHC) funding.
* High-level planning and organisational skills, with ability to manage competing priorities.
* Strong communication and interpersonal skills, with ability to build rapport with clients, families and healthcare professionals.
