We're partnering with a highly respected national organisation to recruit an experienced Clinical Care Coordinator for a rewarding role supporting individuals and families with complex mental health and psychosocial needs.
This position is ideal for a qualified mental health professional who thrives in a client-facing, case management environment and is passionate about making a meaningful impact in the community.
Your new role
This role is all about helping people-especially those dealing with mental health challenges-get the support they need.
You'll work one-on-one with clients who might be struggling with things like anxiety, trauma, or life stress, and help them find the right services, care, and tools to feel better and move forward.
You won't be doing long-term therapy, but rather short-term support to get people back on track. You'll talk with them, understand what's going on, and help them make a plan. You might also work with their families, other health professionals, and community services to make sure everything is connected and working smoothly.
* Deliver short-term, goal-oriented clinical case management and brief interventions.
* Conduct intake assessments, formulate care plans, and manage risk.
* Collaborate with internal teams and external service providers to ensure coordinated support.
* Maintain accurate client records and documentation.
* Participate in professional development, supervision, and service improvement initiatives.
* Support group programs and community engagement activities.
What you'll need to succeed
* Strong clinical experience in mental health, trauma-informed care, and case coordination.
* Excellent communication and interpersonal skills.
* Ability to manage complex cases and respond calmly to critical incidents.
* Proficiency in clinical documentation and IT systems.
Essential Criteria
* Australian Citizenship.
* Ability to obtain a baseline security clearance.
* Hold or be eligible for a Working with Vulnerable People or Working with Children check.
* Relevant qualifications and registration:
* AHPRA-registered Psychologist, Occupational Therapist, or Registered Nurse,
OR
* Accredited Social Worker (AASW),
OR
* Masters-level Counsellor registered with PACFA (Clinical) or ACA (Level 3 or 4).
