Job Title: Pediatric Palliative Care Social Worker
The Pediatric Palliative Care Social Worker plays a vital role in delivering emotional support, crisis intervention, care planning, and inter-agency coordination to children and their families.
Key Responsibilities:
* Provide psychosocial support and assessment to children and/or families through one-on-one sessions.
* Attend weekly IDT meetings, conduct home visits, and maintain liaison with external stakeholders.
* Advocate for children/families and provide ongoing education and support to staff regarding social work in pediatric palliative care.
Requirements:
* Tertiary qualifications in Social Work and eligibility for membership of the Australian Association of Social Workers (AASW).
* Minimum 3 years clinical experience demonstrating skilled expertise and knowledge in Pediatric Palliative Care.
* Eligibility to drive and willingness to travel for work purposes.
