Job Description
The Aged Care Support Worker plays a vital role in providing essential support services to older people, enabling them to maintain their independence and enhance their well-being.
This role focuses on delivering person-centred care and support in line with individual care plans and aged care standards. Key responsibilities include:
* Direct Client Support: Assisting clients with domestic tasks, mobility, transfers, and transport; offering companionship and encouraging social engagement to reduce isolation and improve quality of life;
* Providing personal care, monitoring and reporting changes in client condition or needs, and encouraging clients to maintain their personal independence;
* Responding to incidents and administering first aid if required.
Key Responsibilities Continued
* Compliance & Documentation: Maintaining accurate records of services provided using digital systems or documentation tools; following workplace health and safety procedures, infection control measures, and manual handling guidelines;
* Teamwork & Communication: Collaborating with team members, health professionals, and families to provide holistic support; maintaining professional and respectful communication with clients, families, and stakeholders;
* General Responsibilities: Upholding the Code of Conduct, Core Values, policies, and procedures; ensuring compliance with the Aged Care Code of Conduct and Aged Care Quality Standards.
About You
Applicants should possess relevant qualifications, experience working in the health/aged care/disability sector, and a genuine desire to provide high-quality support. They should also demonstrate respect and understanding of the health and social issues impacting upon Aboriginal and Torres Strait Islander people in the area.
