Job Description
We are seeking a skilled and compassionate Care and Recovery Coordinator to join our team. This is an exciting opportunity to work with complex and high-risk forensic clients, providing community-based support and case management services.
The successful candidate will have experience working with individuals with drug and alcohol issues and/or offending histories. They will be able to facilitate comprehensive assessments, treatment suitability assessments, assertive referral and linkage with other services as appropriate, and provide individual client support including provision of harm reduction, psychoeducation, motivational interviewing, bridging support and risk management.
This role requires the ability to work in a fast-paced dynamic environment, prioritise tasks, and manage a shared caseload. The ideal candidate will have excellent verbal and written communication skills, and the ability to build and maintain positive working relationships with internal and external stakeholders.
Key Responsibilities:
* Facilitate comprehensive assessments and treatment suitability assessments for high-risk forensic clients.
* Develop and implement individualised care plans to meet the needs of clients.
* Provide assertive referral and linkage with other services as appropriate.
* Offer individual client support including provision of harm reduction, psychoeducation, motivational interviewing, bridging support and risk management.
* Work collaboratively with internal and external stakeholders to achieve positive outcomes for clients.
