Job Overview:
We are seeking dedicated individuals with lived experience of mental ill health and recovery to provide peer support in an inpatient setting and post-discharge as part of the 28-day Expanding Post Discharge Support initiative.
About the Role:
As a Consumer Peer Worker, you will leverage your personal journey to help others build understanding, hope, and coping skills towards personal growth and resilience. You will contribute from a peer perspective to effective discharge planning processes, promoting successful discharge and reduction in readmission rates by fostering community engagements/supports, education, and support to consumers, referrals, and follow-up after discharge.
Key Responsibilities:
* Provide lived experience peer support to consumers in both inpatient and post-discharge settings
* Contribute to effective discharge planning processes to facilitate successful discharge and reduced readmission rates
* Promote community engagement, educational support, referrals, and post-discharge follow-up
Requirements:
* Experience of mental ill health and recovery
* Ability to share your lived experience with others in a way that builds understanding, hope, and coping skills
* Full driver's license is essential
