About the Role
As an AOD Intake and Assessment Practitioner, you will be the first point of hope for individuals navigating the challenges of alcohol and other drug use. You will provide critical support and build tailored pathways to recovery.
This role is a Permanent, Part-time (30.4 Hours) position, located in Coburg North Victoria.
The successful candidate will have relevant experience in a social service environment and relevant AOD qualifications and experience.
AOD Skillset or willingness to undertake within the first 12 months of commencing employment is highly desirable.

Key Responsibilities
* Be the Guiding Voice: Ensure referrals are responded to swiftly and compassionately, with clear communication to clients and seamless coordination with referring partners.
* Provide Vital Support: Offer brief interventions, risk management, and linkages to essential services like housing, mental health, and crisis support to individuals waiting for treatment.
* Conduct Meaningful Assessments: Build trusting relationships, identify individual needs and risks, and create tailored, outcome-focused care plans.
* Champion Collaboration: Work closely with families, partner agencies, and clinical teams to ensure a united, wraparound response that uplifts every client's journey.
* Uphold Excellence: Maintain accurate records, engage in team-based learning, identify service gaps, and contribute to a culture of continuous improvement.
